29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:               On or on dates prior to May 7, 2013-  Employer failed to provide step-by-step LOTO specific procedure training to the their maintennace personnel.  Employees were exposed to crushing or struck-by hazards when working with balers, trash trucks and press break equipment.

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).   On or on dates prior to May 6, 2013 at 2166 US Highway 59, Tarkio, Missouri- It was discovered during inspection that the employer had not ensured each employee operating a powered industrial truck had completed the required training.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  On, or at dates prior to, May 7, 2013- Maintenance Area-  Employees were exposed to struck-by hazards when working with a bench grinder which did not have the work rest adjusted within one eighth inch.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  On, or at dates prior to, May 7, 2013-Maintenance Area- Employees were exposed to a struck by hazard when working with a bench grinder which did not have the tongue guard adjusted within one-fourth inch.

29 CFR 1910.305(a)(2)(ix): All lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard. Brass shell, paper-lined sockets, or other metal-cased sockets may not be used unless the shell is grounded.   On or on dates prior to May 7, 2013- Employees working at American Recycling in the maintenance shop were exposed to an electric shock hazard in that the lamp used on the grinder was not protected by a guard.
