29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) At the Warehouse, on or about and at times prior to May 09, 2013, the employer added an attachment (cloth lift) to the forks to hoist and hold rolls of fabric in place during transfer from wood pallets to storing racks, exposing employees to unstable loads, and struck-by hazards. ABATEMENT CERTIFICATION IS REQUIRED.

29 CFR 1910.178(g)(2): Battery changing and charging installations for industrial trucks were not provided with facilities for flushing and neutralizing spilled electrolyte. a) At the Battery Charging Area, on or about and at times prior to May 09, 2013, means of neutralizing spilled battery acid such as an emergency shower station were not provided for employees who removed batteries from powered industrial trucks and racked them for charging, resulting in a chemical burn hazard. ABATEMENT CERTIFICATION IS REQUIRED.
