OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazard of being struck by the load should the load slip off the hook or the lifting device fails.    a.  Main pour area, on or about 05/16/13:  Two Shepard Niles hoists, (units #4 and #5), one P&H hoist, (unit#1), one Cleveland Tramrail hoist, (unit #2) and one Shepard hoist, (unit #5), were being operated without hook throat safety latches.    Among other methods, one feasible and acceptable method to correct the hazard is to:    Install new safety latches on the lifting hooks in accordance with ANSI B-30.16-1993, section 16-1.2.9.    b.  Main pour area, on or about 05/16/13:  Cleveland Tramrail hoist (unit #2) below-the-hook lifting device was not labeled with its load rating capacity.    c.  Scrap charge storage area, on or about 05/16/13:  Below-the-hook lifting device uses to weigh the charge carts was not labeled with its load rating capacity.    Among other methods, one feasible and acceptable method to correct this hazard is to:    Mark the rated load lifting capacity on the below-the-hook lifting device in accordance with ASME B30.20-1993, section 20-1.2.1.    d.  Cupola Area, on or about 05/16/13:  A pneumatic powered, hydraulic freight elevator was being used without any hoist way doors/gates.    Among other methods, one feasible and acceptable method to correct this hazard is to :    Install and/or repair the doors and install interlocks in accordance with ASME A17.3-1993, Safety Code for Existing Elevators and Escalators.

Platform at top of bucket elevator, one side of the platform was not equipped with a standard guardrail system.    f.
