29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s) (a) Laundry, Building 59; When the laundry mechanic was using his assigned tags, identification tags were not available to laundry staff while performing lockout-required duties such as, but not limited to, cleaning the filter on the Vision Image LA steamer. The St. Cloud VA Health Care System was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 92592, notice number 1, item number 5a, and was affirmed as a final order on May 2, 2012, with respect to a workplace located at 4801 Veterans Drive, St. Cloud, MN.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: (a) Dietetics Dish Room 10B, Building 50; the lockout/tagout procedure checklist that employees are required by the employer to use in order to document completion of each step contained an incorrect location for the disconnect because the procedure checklist was for the dish room in Building 4.
