29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: On or about May 8, 2013, at the worksite located at 2208 Race Street in Searcy, Arkansas, a midrail was not provided on the end of the guardrail system surrounding the JLG Lift, 1923E/ML6067, work platform. Employees working from the elevated platform attaching studs to the roof deck were exposed to an approximately 15 ft. fall to ground surface below.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about May 8, 2013, at the worksite located at 2208 Race Street in Searcy, Arkansas, employees elevated approximately 15 ft. in a lift basket to a New Holland extensible boom Sky Trak, s/n 14508, did not wear fall protection while preparing to frame parapet walls. This condition exposed employees to tip-over and falls from elevation.

This condition exposed employees to falls from elevation.
