29 CFR 1910.243(d)(4)(xiii): All tools shall be used with the correct shield, guard, or attachment recommended by the manufacturer. Facility - On or about May 09, 2013 employee used a Duofast staple gun to drive 7/16 inch x 1 1/2 inch 16 gauge staples into a wooden furniture frame. The contact safety tip was not installed on the driver.

29 CFR 1904.29(a): Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report. Facility - On or about May 09, 2013 facility did not keep a Log of Work-Related Injuries and Illnesses, OSHA form 300 and Summary of Work-Related Injuries and Illnesses, OSHA form 300-A.
