Employee was working on a makeshift scaffold 30 feet above the next lower level without any fall protection. Observed on or about 10 May 2013.

Center hallway access stairway to upper levels were being used by employees without stairrail system installed. b) 24 Woodside Avenue, Elmsford, New York. Rear exit stairway used as an entry/exit point by employees did not have stairrails on the unprotected edge.
