29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.        a) 24 Woodside Avenue, Elmsford, New York. Employee was working on a makeshift scaffold 30 feet above the next lower level without any fall protection. Observed on or about 10 May 2013.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)     a) 24 Woodside Avenue, Elmsford, New York. Employees were working on a residential roof without fall protection while working 30 feet above the lower level. Observed on or about 10 May 2013.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:      a) 24 Woodside Avenue, Elmsford, New York. Center hallway access stairway to upper levels were being used by employees without stairrail system installed.     b) 24 Woodside Avenue, Elmsford, New York. Rear exit stairway used as an entry/exit point by employees did not have stairrails on the unprotected edge.
