29 CFR 1926.152(g)(11): Each service or refueling area was not provided with at least one fire extinguisher having a rating of not less than 20B:C located so that an extinguisher would be within 75 feet of each pump, dispenser, underground fill pipe opening and lubrication or service center.    (a) Phoenix Contact Building, Diesel Tank - The employer did not ensure that a 20 B:C fire extinguisher was provided at the company's diesel fuel tank, employees refueled a JLG/LULL 955E-42 telehandler and a Gradall 544D-10 telehandler from the diesel fuel tank and were exposed to burn hazards on or about May 10, 2013.  (b) Phoenix Contact Building, Mortar Mixer - The employer did not ensure that a 20 B:C fire extinguisher was provided at the company's mortar mixing station, an employee refueled the gasoline powered mortar mixer and was exposed to burn hazards on or about May 10, 2013.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  a) Pheonix Contact Building - Masons worked from a Fraco 20 K scaffold system and were not protected by an end rail on the south end of the platform.  Employees were exposed to a fall of 19 feet from the scaffold to the ground below on or about May 10, 2013.    No abatement certification or documentation required.
