29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illness (OSHA Form 300 or equivalent) was not completed in the detail required by the regulation. a. At the work site located at 11351 Lafayette-Plain City Road, Plain City, Ohio, at the time of the inspection, the employer had not recorded information in detail with a description of the injury or illness and the substance/object that made the person ill as required by the standard in column (F) on the 2013 OSHA 300 Log of Work-Related Injuries and Illnesses. An employee had been injured on April 30, 2013. Abatement certification required within ten days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.7(b)(4): How do I record a work-related injury or illness that results in restricted work or job transfer? When an injury or illness involves restricted work or job transfer but does not involve death or days away from work, you must record the injury or illness on the OSHA 300 Log by placing a check mark in the space for job transfer or restriction and an entry of the number of restricted or transferred days in the restricted workdays column. a. At the work site located at 11351 Lafayette-Plain City Road, Plain City, Ohio, the employer did not record a work place injury involving restricted work by placing a check mark in column (I) and an accurate number of days in column (K) for injured employee #2 on the 2013 OSHA 300 Log of Work-Related Injuries and Illnesses. Abatement certification required within ten days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
