29 CFR 1910.26(c)(1): Good safe practices in the use and care of ladder equipment were not employed by the users to get maximum serviceability, safety, and to eliminate unnecessary damage of equipment: (a) Employees were exposed to an approximate 9' fall hazards while using a 12' metal step ladder and the employer did not ensure that the ladder was not placed in front of a door.
