29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):   On or about May 10, 2013, the loft area, approximately eight (8) feet five (5) inches above the office and break area used for storage was not guarded with standard railing, exposing employees to a fall hazard.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that every open-side floor platform four (4) feet or more above adjacent floor or ground level shall be guarded by a standard railing or its equivalent.

29 CFR 1910.243(c)(3): Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel:   On or about May 8, 2013, a portable angle grinder was used without guarding, exposing employees the hazards of being caught in the spinning wheel or struck by debris of an exploding wheel.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that grinders are used with their guarding in place.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:   On or about May 10, 2013, flexible 110 volt orange extension cords were used as permanent wiring to power two receptacle boxes in the maintenance area, exposing employees to an electrical shock hazard.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that flexible cords are not affixed to building surfaces and used as permanent wiring.

Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating are repaired or replaced.

Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all pull boxes, junction boxes, and fittings are equipped with the appropriate covers and are installed so that there are no gaps or openings.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  On or about May 10, 2013, the flexible cords in the facility show signs of strain relief issues.  Employees were exposed to an electric shock hazard.     a:  The outer sheathing of the cord for the Dewalt angle grinder fitted with a wire brush had gaps where the cord met the plug and where the cord met the tool exposing the inner wiring.     b:  The outer sheathing of the cord for the Black and Decker angle grinder fitted with a grinding wheel had a gap where the cord met the tool exposing the inner wiring.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that flexible cords are provided with strain relief.
