On or about May 8, 2013, Building Roof, Employees did not wear protective helmets when working below a crane load ball and hook used to hoist a condenser which exposes employees to struck-by hazards.

29 CFR 1926.1428(a)(3): The employer did not make the documentation for whichever option is used available at the site while the signal person is employed by the employer. The documentation did not specify each type of signaling (e.g. hand signals, radio signals, etc.) for which the signal person meets the requirements of paragraph (c) of this section. a. On or about May 8, 2013, Building Roof, Crane signal person qualification was not documented for an employee who signaled a truck crane to hoist a condenser which exposes employees to struck-by hazards.
