Sufficient unannounced inspections and unannounced follow-up inspections were not conducted by the agency to ensure the identification and abatement of hazardous conditions: Employees were not protected from fall hazards while performing maintenance duties on metal roofs with unguarded skylight panels. The agency did not conduct inspections in all rooftop workplace areas such as the buildings with metal roofs.

29 CFR 1910.27(f): All ladders were not maintained in a safe condition. All ladders were not inspected regularly, with the intervals between inspections being determined by use and exposure. At building 1260, employees were exposed to slip and fall hazards up to 20 feet above the ground level when accessing the roof using a permanent, fixed ladder with the following deficiencies: A). The rungs were not coated with non-slip coating. B.) The top rails were made of angle iron which projected into the climbing path.
