29 CFR 1926.451(e)(1): Safe access was not provided to scaffold platforms when the platforms were more than 2 feet (0.6 m) above or below a point of access:         a) Employees were not provided with safe access to scaffolding from which they were required to work at an elevation of approximately 22 feet.        In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1926.452(k)(1): Ladder jack scaffold platforms exceeded a height of 20 feet (6.1 m):       a) Employees were exposed to the hazard of falling approximately 22 feet during the performance of work from a ladder jack scaffold that exceeded the 20 foot maximum permissible height for ladder jack scaffolding.
