29 CFR 1926.100(a):  Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:       a)  On December 21, 2016, employees  were not wearing protective helmets where there was a possible danger of head injury.       Presidential Construction & Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 958913, citation number 1, item number 1 and was affirmed as a final order on June 11, 2014, with respect to a workplace located at 696 Prospect, Winnetka, IL 60093.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a)  On December 21, 2016, employees engaged in roofing activity were not protected from fall hazards while working at an elevation of approximately 27 feet above the ground.    Presidential Construction & Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1047289, citation number 2, item number 1 and was affirmed as a final order on June 23, 2015, with respect to a workplace located at 503 W. Cedar St., Arlington Heights, IL 60004.     Presidential Construction & Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 958913, citation number 2, item number 1 and was affirmed as a final order on June 11, 2014, with respect to a workplace located at 696 Prospect, Winnetka, IL 60093. 29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:      a) On December 21, 2016, employees  were exposed to the hazard of falling while working from ladder jack scaffold platform elevated approximately 17 feet above the ground.      Presidential Construction & Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(I), which was contained in OSHA inspection number 906327, citation number 1, item number 2 and was affirmed as a final order on June 19, 2013, with respect to a workplace located at 3738 Salem Walk, Northbrook, IL 60062.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
