29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. a) 400 E Schuylkill Road, Bellewood Country Club, construction hole adjacent to swim club driveway - On or about 5/10/2013, an employee performing security duties fell into a construction hole approximately 5 1/2 feet deep and 2 feet in diameter that was not protected by a cover or other means.
