29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: On or about 5/13/2013, at the above addressed jobsite, the employer did not ensure the following. a. The employer did not inspect the fall protection equipment used by two employees prior to conducting work. b. The employer did not ensure defective fall protection equipment with tears, snags, and signs of corrosion, was removed from service thus exposing an employee to a fall hazard. PLEASE NOTE: Certification and documentation of abatement are required for this violation.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: On or about 5/13/2013, at the above addressed jobsite, three employees were exposed to a free fall hazard of more than 6 feet while attached to personal fall arrest systems that involved a 3 foot Safe-T-Strap, 9 foot Safety line, and a 6 foot lanyard.. PLEASE NOTE: Certification and documentation of abatement are required for this violation.
