29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   (a)Production Area-Authorized employees were exposed to unexpected energy start up hazards on or about 5/10/13.  The employer did not develop and implement procedures for each piece of equipment.  The authorized employees are required to set up, perform maintenance, and make repairs on the following machines:   a) Mazak Hypergear Laser  b) Finn Power C6 Turret  c) Finn Power C5 Turret d) Finn Power F6 Turret e) Amada HDS 220 Press f) Amada HD 130 Press  g) Amada HDS 130 Press h) Donaldson Dust Collector  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:   (a)Production Area-Employees were exposed to burn hazards in that the fire extinguishers in the weldng area were not mounted on or about 5/10/13.  (b)Production Area-Employees were exposed to burn hazards in that the fire extinguisher in the power press area was not mounted on or about 5/10/13.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
