29 CFR 1926.501(b)(12): 29 CFR 1926.501(b)(12) Each employee engaged in the erection of precast concrete members (including, but not limited to the erection of wall panels, columns, beams, and floor and roof "tees") and related operations such as grouting of precast concrete members, who is 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems. a. 1600 Harbor Boulevard, Weehawken, NJ - Building A, West end, 8th floor roof Employees were performing concrete plank (precast concrete) installation with unprotected edges greater than 6 feet above the next lower level were not protected from falling by use of a guardrail system, safety net system, or personal fall arrest system. Employees were exposed to fall hazards. Violation observed on or about May 13, 2013 b. 1600 Harbor Boulevard, Weehawken, NJ - Building A, West end, 8th floor roof. Employees were performing concrete plank (precast concrete) installation with unprotected edges greater than 6 feet above the next lower level were not protected from falling by use of a guardrail system, safety net system, or personal fall arrest system. Employees were exposed to fall hazards. Violation observed on or about May 14, 2013 Say-Core, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1) which was contained in OSHA inspection number 417083, citation number 01, item number 01, and was affirmed as a final order on March 22, 2013 with respect to a workplace located at 4837 Lake Road, Route 19, Brockport, NY, 14420. Say-Core, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(12)which was contained in OSHA inspection number 316182633, citation number 01, item number 02b, and was affirmed as a final order on April 16, 2012, with respect to a workplace located at 3605 Georgia Avenue NW, Washington, DC, 20009.
