29 CFR 1926.452(k)(2): All ladders used to support ladder jack scaffolds shall meet the requirements of Subpart X of this part -- Stairways and Ladders, except that job-made ladders shall not be used to support ladder jack scaffolds: 13728 Decatur Road, Omaha, NE 68152 (Job-site): The employer did not ensure that two ladders were used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support). The top support of the ladder was approximately 8 ½ feet in height and approximately 3 ½ working length, creating an angle of approximately two-fifths.
