29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:   a) West Side of Phoenix Contact Building - Employees worked from a suspended scaffold to install the frames for the exterior windows.  The 110 VAC power cords for the scaffold's two Skyclimber 750 winches were connected inside a metal 2 gang box and the cords were zip tied to the mid rail of the scaffold.  The box was not rated for wet locations and was listed for mounting to a secure surface.  Employees were exposed to the risk of electrical shock on or about May 10, 2013.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
