A) 155 Hobart Avenue, Summit, NJ 07901: Employees performing exterior trim work at heights of approximately 18' from the ground below on a bracket scaffold platform were not provided with a fall protection system. The bracket scaffold work platform was missing mid and end rails. Condition observed on or about May 14, 2013.
