29 CFR 1910.119(l)(1): The employer did not establish and implement written procedures to manage changes to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process. In the Catalytic Hydrodesulfurization Unit #2 (CHD-2) at ExxonMobil Beaumont Refinery, the employer did not ensure that changes made while performing procedures for removing hazardous energy from process equipment in the CHD-2 Unit of the ExxonMobil refinery in Beaumont, Texas were updated to reflect current configurations, practices, tasks and equipment setup. a) Steps 22 through 26 of the Chemical Wash CHD-2 for Opening and Mechanical Work procedure were not performed during the cleaning of the E-1 exchangers, was not updated and did not reflect the required steps. b) Portable heat exchanger was not used to control the wash solution temperature (step 4.6 of procedure not performed) c) High point vents were not used to verify that the system was filled with water for the wash (step 4.3 of procedure not performed) d) One of the key project indicators for hydrocarbon and scale removal, the circulation flow rate of 300 - 500 gpm was not verified as required by the procedure. e) The required amount of water needed was not injected into the system to adequately wash the system free of hydrocarbons.
