29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:     a) 457 Van Houten Ave., Passaic Park, NJ: Employees allowed to use portable fire extinguishers to put out a fire were not provided information with training on fire extinguisher use and the hazards of fire fighting, on or about 5/9/2013.

29 CFR 1910.1200(h)(1):     Employers did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area and chemical-specific information was not always available through labels and material safety data sheets     a) 457 Van Houten Ave., Passaic Park, NJ: Employees that worked with chemicals including, but not limited to, DAP Contact Cement were not provided information nor training on the hazards involved with chemical use, on or about 5/9/2013.
