29 CFR 1926.201(a): Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers shall conform to Part VI of the Manual on Uniform Traffic Control Devices, (1988 Edition, Revision 3 or the Millennium Edition), which are incorporated by reference in 29 CFR 1926.200(g)(2): At this site adjacent to the Carancahua St, employees working around heavy equipment and excavation installation, were moving around a high speed curve and were exposed to traffic collision without wearing reflective (highly visible) gear.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i): At this site, employees installing and adjusting electrical conduit piping were working in an excavation 5 feet 7 inches deep with vertical walls and no cave-in protection in place.
