29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k):  (a)  In the Wall Assembly Area, on or about May 21, 2013 and at times prior to, the employer did not train an employee who was exposed to noise at 228% of the action level (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of 90.9 dBA during the 404 minute sampling period, resulting in a noise hazard.  (b)  In the Truss Area, on or about May 21, 2013 and at times prior to, the employer did not train an employee who was exposed to noise at 643% of the action level (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of 98.4 dBA during the 411 minute sampling period, resulting in a noise hazard.  (c)  In the Prep Area, on or about May 21, 2013 and at times prior to, the employer did not train an employee who was exposed to noise at 121% of the action level (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of 86.3 dBA during the 428 minute sampling period, resulting in a noise hazard.  (d)  In the Top Hatch Area, on or about May 21, 2013 and at times prior to, the employer did not train an employee who was exposed to noise at 202% of the action level (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of 90.0 dBA during the 433 minute sampling period, resulting in a noise hazard.  (e)  In the Vane Area, on or about may 21, 2013 and at times prior to, the employer did not train an employee who was exposed to noise at 132% of the action level (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of 86.9 dBA during the 445 minute sampling period, resulting in a noise hazard.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  (a)  In the blasting/painting area, on or about May 21, 2013 and at times prior to, the employer did not provide a medical evaluation to determine the employee's ability to use a respirator for an employee who was required to wear a respirator while sand blasting and painting, resulting in a respiratory hazard.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a)  Throughout the facility, on May 21, 2013 and at times prior to, the employer did not provide hazard communication training for employees who used hazardous chemicals including but not limited to Carbozinc 859 Part A and p-Xylene, resulting in a chemical hazard.
