29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. Employees were working from a scaffold platform that did not have a completed guardrail system in that the top rail was missing on the outside. Employees were exposed to a fall hazard of approximately fifteen (15) feet.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met. The employer utilized items such as Finestone A/BC 1-Step Ready-Mix Stucco product and portland cement manufactured by The Monarch Company and had not developed and implemented a hazard communication program. Employees were exposed to the hazard of not having the required information available.
