(a) Rocky Mountain Rebar; 686 Industrial Blvd., Delta, CO 81416; On and before May 15, 2013, the exposing employer did not ensure that empty 55 gallon drums stacked on their sides up to a height of 4 or 5 barrels in height were stable and secure against sliding or collapse. Condition exposed employees stacking and removing barrels for use to struck by and caught between hazards from the barrels.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury was not guarded. (a) Rocky Mountain Rebar, 686 Industrial Blvd., Delta, CO 81416; On and before May 15, 2013, the exposing employer did not guard the point of operation of two table rebar benders. Condition exposed employees utilizing the table bender to point of operation hazards as the table bender was used to bend and shape rebar. Condition exposed employees utilizing the machine to pinch point and laceration/amputation hazards as they use the machine to bend rebar. (b) Rocky Mountain Rebar, 686 Industrial Blvd., Delta, CO 81416; On and before May 15, 2013, the exposing employer failed to adequately guard the point of operation of a Rebar Shearline Machine, specifically in the rear of the machine at where the point of operation is accessible. Condition exposed employee's utilizing the rebar bending machine to point of operation hazards as they use the shear. (c) Rocky Mountain Rebar, 686 Industrial Blvd., Delta, CO 81416; On and before May 15, 2013, the exposing employer failed to guard the point of operation of an Xcel Ironworker, specifically the punch portion of the ironworker. Condition exposed employee's utilizing the ironworker to point of operation hazards as they used the ironworker.

(a) Rocky Mountain Rebar, 686 Industrial Blvd., Delta, CO 81416; On and before May 15, 2017, the exposing employer failed to adequately guard the lower portion of an 8.25", 2.5 HP radial arm saw used to cut lumber. Condition exposed employees using the saw to point of operation hazards from the exposed lower portion of the blade.

(a) Rocky Mountain rebar, 686 Industrial Blvd., Delta, CO 81416; On and before May 15, 2013, the exposing employer did not ensure that an adjustable stop was provided on a radial arm saw to prevent the forward travel of the blade to go beyond the position necessary to complete the cut in repetitive steps. Condition exposed employees utilizing the saw to laceration and amputation hazards as the saw blade traveled beyond the end of the table on which the cut was being performed.
