29 CFR 1910.67(c)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. a. On or about May 7, 2013 - at the above addressed jobsite, an employee performing electrical maintenance to a display sign from a Vehicle-mounted Telescopic Boom Aerial Lift basket did not have a body belt and a lanyard secured to the basket, thereby exposing the employee to the hazards associated with a fall of approximately twenty (20) feet.

29 CFR 1910.132(f)(1): The employer shall provide training to each employee who is required by this section to use PPE. Each such employee shall be trained to know at least the following: a. On or about May 7, 2013 - at the above addressed jobsite, the employer failed to provide a training program on the appropriate Personal Protective Equipment (PPE) for each employee who might be exposed to fall hazards.
