29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  (a) Shop: On or about 16 May 2013 and at times prior thereto, when the employer required employees to wear a respirator such as but not limited to a North 7700 half-face respirator with organic cartridges or a 3M 6300 half-face respirator with organic cartridges, the employer did not ensure employees were provided with and/or implement an effective written respirator program that included the following provisions:  1.  The employees had not been medically evaluated to wear a respirator.  2.  The employees had not been annually fit tested on the tight fitting respirators.   3.  Correct procedures for Maintenance and Care of respirators.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a) Dip Tank Area: On or about 1 April 2013 and at times prior the employer did not provide a working eye wash station near the dip tank that contains an 11.0 pH solution of  Sodium Hydroxide used to clean equipment prior to repair.
