29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) For the metal fabrication facility at 21228 Powell Rd., Brooksville, FL 34604, as observed on or about 05/16/2013, a manual lathe was used at the facility daily without a guard to protect employees from rotating parts and flying metal shavings. Employees were exposed to burns from hot, flying metal shavings.

29 CFR 1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work: a) For the metal fabrication facility at 21228 Powell Rd., Brooksville, FL 34604, as observed on or about 5/16/2013, multiple abrasive wheel bench grinders were missing their tool rests at the establishment. The gap between abrasive wheels and the peripheral guard at the location of the missing tool rests was measured to be up to 2 inches.
