29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. The employer did not change the capacity, operation, maintenance instructions plates, tags, and/or decals: (a) The employer failed to ensure that the forklifts' capacity plates, tags or decals; were modified to reflect the capacity changes related to attachment addition, and to get prior written approval from the manufacturer of the forklifts for the modifications.
