29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:    At the jobsite located at 1870 Mayfair Drive, Omaha, NE:    The employer failed to provide fall protection for employees engaged in residential construction activities. Employees of A-Team Restoration were observed performing roofing installation and were exposed to a fall height greater than 6 feet.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures        to be followed in order to minimize these hazards:     A-Team Restoration:  The employer did not have a training program to ensure that employees minimized hazards when exposed to fall hazards. Employees of A-Team Restoration were observed by a Compliance Officer performing roofing activities at heights greater than 6 feet without using any convention fall protection; none of the employees had received training from their employer.

29 CFR 1926.1053(b)(1):  Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder(s) was/were not secured at its top to a rigid support, and/or a grasping device, such as a grab rail, was not provided to assist the employees in mounting and dismounting the ladder:  Jobsite located at 1870 Mayfair Drive, Omaha, NE:  The employer failed to ensure that ladders used by employees to access the roof extended at least more than 3 feet above the roof and or were secured and a grasping devices provided for a safe access. On the jobsite the employees were using several aluminum ladders to access the roof; none of the ladders extended more that 3 feet beyond the roof line nor were they secured to prevent accidental displacement.

29 CFR 1926.1060(a):  The employer did not provide a training program for each employee using ladders.   A-Team Restoration on the site located at 1870 Mayfair Dr. Omaha, NE:     Employees of A-Team Restoration were not trained by their employer to recognize hazards related to the unsafe uses of ladders. During the inspection the CSHO observed employees several extension ladders to access the roof. The ladders did not extend a minimum of 3 past the level being accessed, nor were they secured to prevent displacement.
