Loft Area of Warehouse:  On March 14, 2013, the two sections of the railing on the loft were hinged to swing open out over the loft and was not secured with latches provided.  Both sections had an additional piece of unsecured railing placed in front of the hinged area that did not withstand weight in the direction over the loft because they were not anchored to the floor.
