29 CFR 1926.502(d)(6)(i): A snaphook of the locking type was engaged directly to webbing, rope or wire rope despite not being designed for that purpose. (a) Location - 400 Memorial Drive, Cambridge, MA: On or about 5/16/2013, employees were exposed to the hazard of falls while using personal fall arrest systems the anchorages for which were vertical lifelines tied back upon themselves with snaphooks.
