29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: On May 16, 2013 an employee was not protected form a fall while working from an elevated conveyor, exposing the employee to a fall of 13.6 feet above grade. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
