29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. In the production building, the employer did not ensure that all safety lines were installed on theTas 10 Color Screen Printing Machinee Serial No. 205 XIE-2P764, exposing employees to struck by hazards from rotating parts.  b. In the sublamation building, the employer did not ensure that employees pulling laces through the rollers on the Sublimation Machine, to the sorting bins were protected from rotating parts. Employess reached between the rollers to retrieve the laces.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a. In the sublimation building, the employer did not ensure that the hot cutting machine used to cut laces was operated with the point of operation guards in place. The machine was operated while the safety guards and interlock systems were by-passed, resulting in an amputation to employees finger.  b. In the sublimation buiding, the employer did not ensute that the hot cutting machine used to cut velcro was operated with the point of oeration guards in place. The machine was opearted while the safety guards and interlock systems were by-passed.
