29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:     a/ Frank Millman/Edison NJ Site - Warehouse:   1/ Metal storage racks (including but not limited to C-Aisle) had product stored that was not blocked, shrink wrapped or other-wise secured to prevent falling.  2/ Damaged wooden pallets/skids were not all replaced and were continued to be used.  Some of them were double stacked with product stored on them to compensate for the damage.     Observed on/or about 05/16/2013.

29 CFR 1910.178(l)(4)(iii):  An evaluation of each powered industrial truck operator's performance was not conducted at least once every three years:    a/ Frank Millman/Edison NJ Site - Warehouse:   An evaluation of forklift training has not done in excess of 3 years. Documentation to show initial training was not available.  Employees are required to operate electrically powered "Reach" and "Standard" forklift trucks and electrically powered hand jacks -"Walk-Behind" type.    Observed on/or about 05/16/2013.
