29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Workplace- The employer did not develop or utilize energy control procedures such as Lock Out/ Tag Out device application for the dissipation of energy and verification of isolation on machines such as, but not limited to the Pacific Hydraulic Press Shear, model, J110-12H, serial # A550 and Chicago Dries and Krump, model 1012-B, serial #L-167786 and Engel Industries Lockformer Machine, model # 800-S, serial #3101-86; on or about 5/16/13.     Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:    a) Workplace, Ductwork Production Area- Employees who perform die changes and adjustments on machines such as the Pacific Hydraulic Press Shear, model J110-12H, serial #A550 and the Chicago Dries and Krump Press, model 1012-R, serial # 167L86 and, Engel Industries Lockformer, model #800-5, serial #3101-86 did not receive  Lockout/Tagout training; on or about 5/16/13.    Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel:    a) Workplace, Ductwork Production Area- circuit breaker boxes on east wall, housing power to the production area including, but not limited to warehouse lights, press brake machines, plasma cutting machine and roll forming machine was blocked by an approximately 4'x7' Liner Cutter Machine; on or about 5/16/13.    b) Workplace, Ductwork Production Area- circuit breaker box in center of production area housing power including, but not limited to warehouse lights and roll forming machines was blocked by a TDC Roll Former Machine; on or about 5/16/13.    Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
