29 CFR 1926.304(d): Portable, power-driving circular saw(s) were not equipped with a guard above and below the base plate or shoe:                                                                                                        This violation most recently occurred on May 16, 2013, at a commercial building under construction located at 3726 Towne Crossing Boulevard, Mesquite, TX 75150; where at least one (1) employee used an electric circular saw that had a wood chip wedged in the guard, rendering the guard ineffective.  The employee was exposed to amputation and severe laceration hazards.
