29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: (a) On May 17, 2013, at the River City T's Facility, employees were exposed to serious injuries, in that the 240 Volt Electrical Panel located in the NW Corner of the warehouse had knock out in slot 14 opening that had not been effectively closed. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
