29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees: On or about May 13, 2013, at the new PRARNG Readiness Center, Fort Buchanan, PR. Employees were not provided with a ladder when working inside a trench during underground water line operation. The exposed employees were observed working inside a trench that was 4 1/2 feet deep, 15 ft long and 4 ft wide without a ladder while removing an existing 24 inch water pipe in order to install a new 36 inch pipe. NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems shall be made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: On or about May 13, 2013, at the new PRARNG Readiness Center, Fort Buchanan, PR. Employees were working inside of an open trench which proper means of egress and protective systems were not made by a competent person. The exposed employees were observed working inside a trench that was 4 1/2 feet deep, 15 ft long and 4 ft wide without a ladder and without respective protective systems in place while removing an existing 24 inch water pipe in order to install a new 36 inch pipe. NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.
