29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At 266 Lee Rd 2111 Phenix City, Alabama: On or about May 14, 2013 and at times prior; the controlling employer failed to ensure a portable extension ladder used on site by subcontracted employees was extended at least three feet above the landing area exposing them to fall hazards of 8.2 feet to the ground below.
