29 CFR 1910.1200(h)(3)(iii): Employees did not take measures to protect themselves from these hazards, including specific procedures the employer has not implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures, and personal protective equipment to be used: On or about April 15, 2013, the employer did not train employees with appropriate work practices and emergency procedures when PdCare enzyme detergent and Patterson Tarter and stain remover were accidentally mixed creating formaldehyde; exposing employees to a chemical reaction in the dental clinic.
