On or about May 17, 2013, an employee, who was instructed to perform a manual hand cleaning task of the Valve sustained a 3-finger amputation injury while trying to perform the task.

29 CFR 1910.147(e)(1): The machine or equipment. The work area shall be inspected to ensure that nonessential items have been removed and to ensure that machine or equipment components are operationally intact. Establishment: The employer did not ensure through providing specific training, instruction and/or visual observation that the work areas in close proximity of the horizontal screw augur were checked and inspected prior to removal of lockout/tagout devices to ensure that all nonessential components, such as (but not limited to) a piece of wood board was not only removed from, but also the Rotary Valve Number One was intact in a safe mode for the employees engaged in servicing, cleaning, and/or maintenance of the Rotary Valve Number One (Valve). On or about May 17, 2013, an employee, who was instructed to perform a manual hand cleaning task of the Valve sustained a 3-finger amputation injury while trying to clean the Valve that was covered with a piece of wood board and not in a safe mode to perform such task.
