29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     On or about May 15, 2013, in the welding shop of the Farley's Inc. facility in Siloam Springs Arkansas, the employer did not guard the unused portion of band saw blades used to cut metal, exposing employees to amputation hazards in the following locations:     a) The band saw in the north end of the welding shop.   b) The ROLL-IN band saw in the east side of the coil room.
