29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation had been performed; the date(s) of the hazard assessment; and, which identified the document as a certification of hazard assessment:  a) In the workplace, on or about May 17, 2013 - The employer did not complete a written hazard assessment which documented the mandatory usage of safety glasses, steel toe shoes, and gloves during manufacturing tasks.

29 CFR 1910.1200(e)(1): Employer did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following:  a) In the workplace, on or about May 17, 2013 - The employer did not develop a written hazard communication program.    The employer handled cement mixtures which included the following products:  Cemex Type I cement which included Portland cement, gypsum, and quartz; limestone gravel; and river sand during pre-cast cement manufacturing.
