29 CFR 1910.132(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used and maintained in a reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  (a)  Dry Sanding Area:  An employee handles large pieces of annealed glass and wears cut resistant sleeves and gloves.  The employee's cut resistant sleeves provided were not long enough to cover the employees lower arm, leaving an a approximately 2 inches of the lower arm exposed between the sleeve and the glove, as observed on May 21, 2013.    Abatement Certification Required for this Item.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   (a)  Tempering Furnace:  Machine guarding was not provided for a motor unit for the end rollers of the Tempering Furnace. Employees were exposed to moving parts and ingoing nip points from the chain and four sprockets on the motor unit, as observed on May 21, 2013.
