29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: Jobsite: 909 Holiday Ave, Hazelwood, MO 63042 Two employees gained access and egress by utilizing a portable ladder which did not extend at least 36" above the upper landing (roof). The side rails measured approximately 18" above the landing. This improperly positioned ladder was located at the northwest eave of the residential structure that employees were sheathing. The fall hazard at ladder landing measured approximately 9 feet, 9 inches.
