29 CFR 1910.252(b)(2)(i)(A): Helmets or hand shields were not used during all arc welding or arc cutting operations, excluding submerged arc welding:  JOB SITE:  1222 Spruce Street, Saint Louis, MO 63103, located at government parking lot entry way, southeast of the building, a worker was observed on a Werner Fiberglass extension ladder, approximately 15 feet above ground level, doing welding work on the frame of the security gate without of any form personal protective equipment (PPE).
